Parent/Guardian Information and Permission Form
In order to participate in a camp at Knock Knock Children’s Museum, a parent or guardian is required to provide contact information for themselves and anyone else who is allowed to pick-up their child from Knock Knock when the camp session is over for the day. We also require some basic medical information on each of our campers along with being given express permission to act as necessary in case of an emergency. Please click the button below, print the form, fill it out completely, and bring it on the first day of camp.
If camps do not meet minimum enrollment, Knock Knock has the right to cancel and refund participants at 100%. Individual refunds are allowed 2 weeks prior to camp start date, less the $50 non-refundable fee. Emergency cancellations with a full refund will be considered on a case-by-case basis.
